This notice explains how to handle health information when you contact MyOfAI.
This website is not a channel for protected health information
Please do not send protected health information (PHI), such as patient names, dates of birth, diagnoses, treatment details, insurance numbers or medical records, through this website. This includes our contact, checklist and waitlist forms, email to our addresses, text messages and phone voicemail. Our forms and email are set up for business contact details only.
If PHI is sent to us by mistake
We will not use it. We will delete it from our systems as soon as we notice it and let you know. Please tell us right away at hello@myofai.com or (786) 520-1462 if you think you sent PHI.
Medical and other covered practices
If your practice is a covered entity under HIPAA and our work would involve access to PHI, we sign a business associate agreement (BAA) with you before we receive or can access any PHI. Until a BAA is signed, our work is limited to equipment and systems that do not contain PHI, or to test data that contains none.
What we do not claim
This website does not claim that MyOfAI, its products or its services meet HIPAA requirements, and nothing on it is a statement of that kind. Each practice remains responsible for its own HIPAA obligations, including risk analysis, policies and staff training. We do not run advertising tags on this page.
